You can access request forms under More Services in Online Banking. Some forms (ie. dispute transactions, make overdraft decisions, and request loan payoff quotes) are available in "Self-Services Center" under the Service tab in the Republic Bank Mobile app.
Update your address, phone number, or email, add travel notes to your debit or credit card, make overdraft coverage decisions, download a direct deposit form, and more.
